Monocular Pendular Nystagmus in Patient with Sporadic Cerebellar Ataxia Syndrome

Abstract
Acquired pendular nystagmus has been traditionally attributed to ¡°Guillain-Mollaret triangle¡± lesions. It is encountered in a variety of clinical conditions, the two most frequent being multiple sclerosis (MS) and oculopalatal tremor (OPT). But we experienced monocular pendular nystagmus in patients who had progressive cerebellar ataxia but showed no palatal tremor. MRI disclosed cerebellar atrophy, but there was no hypertrophic degeneration of the inferior olivary nucleus or focal lesion involving the Gullain-Mollaret triangle. Distuption of the cerebellum that modulate gaze holding may give rise to pendular nystagmus.
